BEST to kick off premium bus service between Thane, Bandra Kurla Complex

 The premium BEST bus service is expected to be on the lines of private cab service providers.


The Brihanmumbai Electric and Supply Transport (BEST), an undertaking of Brihanmumbai Municipal Corporation (BMC), is likely to begin its much-awaited premium services from the second week of December. Lokesh Chandra, General Manager of BEST, said that with five buses, they are set to begin the premium service between Thane and Bandra Kurla Complex.

“We have identified a couple of routes of which the first will be BKC-Thane. Five buses have arrived of which three are in Regional Transport Office (RTO) and are undergoing registration. The other two are at depot and have obtained RTO clearance. These buses will ply between BKC and Thane route in peak hours. In non-peak time, they will ply within BKC as premium service only,” said the IAS officer.

The premium BEST bus service is expected to be on the lines of private cab service providers. The user can book the bus from the exisiting BEST Chalo mobile application, make the payment digitally and board the bus from the nearest stop. The service will be fully air-conditioned, said Chandra, adding the commuters will have a comfortable travelling experience.

On the rationale behind the premium bus service, Chandra said, “Many office goers do not use BEST as they are overcrowded. With option of premium service, these people, who largely prefer private cab as they want to travel comfortably, will also switch to BEST buses. Moreover, the BEST premium service will be affordable unlike cab service providers. It will cost Rs 5 per km, which means a trip on the BKC-Thane route should be in the range of Rs 150 to Rs 200.”

Apart from facilitating the commuters, BEST premium service will also help in reducing road occupancy, according to officials.

The service will have 200 buses via a private operator in phase 1. The fleet will gradually expand to 2,000 buses by 2024.

BEST has tied up with a private operator – Ashok Leyland – on a revenue-share model as part of which the operator will share 4 per cent of earning every month with BEST in return of which it will be allowed to use BEST facilities like bus stops, parking at the depot among others. The drivers of these buses will be provided by the operator.

Maharashtra CM Eknath Shinde sanctions Rs 900 crore for 49 projects in Thane

 THANE: Chief minister Eknath Shinde has okayed a fresh budget for 49 various projects collectively worth Rs 900 crore to be implemented in three administrative wards of Thane city, MLA Pratap Sarnaik informed Wednesday.


As per the plans, the funds for basic infrastructure revamp from the urban development department would be used for strengthening alternate water sources in the area, constructing Lata Mangeshkar music academy, aquarium, swimming pools, revamping gardens, musical fountains at Upwan lake, constructing new roads, community centres, among others.

The works will be taken up in the assembly constituency of Sarnaik that includes Majiwada-Manpada, Vartak Nagar and Lokmanya Nagar areas.Sarnaik said he has spoken with municipal commissioner Abhijit Bangar about drawing a road map for executing the projects at the earliest.

Thane Civic Body's Tax Collection Crosses Rs 500 Crore-mark So Far This Year

 During the corresponding period last year, the tax collection amount was Rs 350 crore, the civic body said in a release.


The Thane Municipal Corporation (TMC) on Wednesday crossed the Rs 500 crore-mark in tax collection so far this financial year, a civic official said.

During the corresponding period last year, the tax collection amount was Rs 350 crore, the civic body said in a release.

Municipal Commissioner Abhijit Bangar said the tax collection so far this year has been Rs 502.12 crore and the maximum collection of Rs 173.98 crore came from the Majiwada-Manpada ward.

“Even though the TMC’s tax collection has crossed the Rs 500 crore-mark, a number of tax payers have not paid the tax so far and they should do it on priority," he said.

IVF Step-by-Step

 The Embryology Laboratory at the Strong Fertility Center is a state-of-the-art facility accredited by the New York State Department of Health. These include in-vitro fertilization (IVF) with standard insemination, intracytoplasmic sperm injection (ICSI), assisted hatching (AHA), embryo cryopreservation, blastocyst culture, TESE and MESA for male factor, and embryo biopsy for pre-implantation genetic diagnosis.


Step-by-Step Descriptions of the IVF Procedures

Step 1: Control Ovarian Hyperstimulation (COH)

COH is done using different protocols. The most common one is a GnRH Antagonist protocol where the secretion of gonadotropin hormones is suppressed in order to prevent premature ovulation. Once optimal suppression is achieved, the next step is the recruitment of multiple follicles by daily injections of gonadotropins. Ultrasound imaging and hormone assessments are used to monitor follicular development. When the lead follicles have reached the appropriate size, the final maturation of eggs is completed by HCG administration. Egg retrieval is scheduled 34 - 36 hours after HCG injection.


Step 2: Egg Retrieval

Egg retrieval is performed in a surgical suite under intravenous sedation. Ovarian follicles are aspirated using a needle guided by trans-vaginal ultrasonography. Follicular fluids are scanned by the embryologist to locate all available eggs. The eggs are placed in a special media and cultured in an incubator until insemination.


Step 3: Fertilization and Embryo Culture

  • If sperm parameters are normal, approximately 50,000 to 100,000 motile sperm are transferred to the dish containing the eggs. This is called standard insemination.

  • The ICSI technique is used to fertilize mature eggs if sperm parameters are abnormal. This procedure is performed under a high-powered microscope. The embryologist picks up a single spermatozoa using a fine glass micro needle, and injects it directly into the egg cytoplasm. ICSI increases the chance that fertilization will occur if the semen sample has a low sperm count and/or motility, poor morphology, or poor progression. If there are no sperm in the ejaculate, sperm may be obtained via a surgical procedure. ICSI is always used to achieve fertilization if the sperm is surgically retrieved.

  • Fertilization is assessed 16 - 18 hours after insemination or ICSI. The fertilized eggs are called zygotes and are cultured in a specially formulated culture medium that supports their growth. They will be assessed on the second and third day after retrieval. If sufficient numbers of embryos exhibit good growth and development, they may be selected to grow to the blastocyst stage in a specially designed culture medium. Blastocyst culture has several advantages. Embryos at this stage have a higher potential for implantation, therefore fewer embryos can be transferred on day 5 to reduce the chance of multiple pregnancies. Low numbers of embryos and poor embryo quality reduce the chances for good blastocyst development. A day 3 embryo transfer is recommended for cycles with low numbers and/or poor quality.

Step 4: Embryo Quality

There are several criteria used to assess the quality of the embryo. This is especially important when trying to decide which embryos to choose for embryo transfer. Early in the morning on the day of your transfer, the embryos are evaluated and photographed by the embryologist. The embryologist and your physician will decide, based on the rate of development and appearance of the embryos, which and how many embryos are recommended to be transferred.

Typically, embryos are transferred at the cleavage stage (day 3 after oocyte retrieval) or at the blastocyst stage (day 5). In the lab, a grading system is used to asses the quality of the embryos.


Day 3 Transfers

Day three embryos are called cleavage stage embryos and have approximately 4 - 8 cells. When analyzing these embryos, we not only look at the number of cells but also how symmetrical they are and whether there is any fragmentation. Fragmentation occurs when the cells divide unevenly, resulting in cell-like structures which crowd the embryo. No fragmentation is preferable but some is acceptable. In our lab, we classify embryos into grades 1 through 4. Grade 1 represents the best quality embryos.


Day 5 Transfers

Day 5 embryos are called blastocyst embryos. At this stage, the embryos have increased in size and are even more developed. They resemble a ball of cells with fluid inside. One of the things we look for at this stage is how expanded these embryos are. The more expanded, the better the quality of the embryo. These embryos are also classified by a number scale, 1 through 6. Grade 6 represents the best quality blastocyst.


Step 5: Embryo Transfer

Embryos are transferred on day 3 when they are at the cleavage stage (6 - 8 cells) or on day 5 when they have reached the blastocyst stage. Embryo transfer is a simple procedure that does not require any anesthesia. Embryos are loaded in a soft catheter and are placed in the uterine cavity through the cervix under ultrasound guidance.


Additional IVF Procedures

Laser Assisted Hatching

An embryo must hatch out of its outer membrane (zona pellucida) before implanting in the uterine wall (endometrium). Sometimes, the zona is abnormally thick. Laser assisted hatching is a technique that allows a small gap in the zona pellucida to be made. This will aid the embryo in breaking out of this membrane and facilitates implantation. It is a technique that is performed before embryo transfer and when doing trophectoderm biopsies. Assisted hatching is specifically recommended for patients who are over 37 years of age, have diminished ovarian reserve as determined by a day 3 FSH level, or have lower antral follicle counts. Patients who are poor responders to gonadotropin stimulation, or have had previous failed implantation may also benefit from this procedure. Studies have shown that assisted hatching improves IVF success rates in both fresh embryo transfers and frozen embryo transfers.


Cryopreservation

Embryo cryopreservation (freezing) may be available to patients that have an excess number of normally fertilized embryos or high quality blastocysts that remain following embryo transfer. Embryos may be frozen at the zygote stage one day after egg retrieval, or on day 5 or 6 at the blastocyst stage.


Microsurgical Epididymal Sperm Aspiration (MESA) or Testicular Sperm Extraction (TESE)

Some patients' semen samples contain no spermatozoa due to a congenital obstruction of the sperm ducts, vasectomy, failed vasectomy reversal, or primary testicular failure. In these conditions, a urologist can obtain sperm surgically from the epididymis (MESA) or from the testis (TESE). This sperm can be frozen and used for fertilization by ICSI.


Embryo Biopsy

Preimplantation Genetic Testing (PGT)

PGT is a procedure that is performed in conjunction with IVF. It is designed to help detect genetic abnormalities/inherited genetic diseases in embryos before implantation, thereby minimizing the transfer of affected embryos.


Preimplantation Genetic Testing - Aneuploidy (PGT-A)

Normal human cells contain 46 chromosomes located in the nucleus of the cell. Chromosomes carry the genetic information in the form of DNA. Every human being receives 23 chromosomes from each parent. If an error occurs leading to the egg or sperm having a missing or extra chromosome, the embryo created will also have a missing or extra chromosome. This condition is called aneuploidy. Most of the aneuploidies will not result in implantation of the embryo, but certain aneuploidies, such as trisomy 21, can implant and lead to Down’s syndrome. Some other common aneuploidies include trisomy 13, trisomy 18, and Klinefelter's syndrome (XXY).


Indications for PGT

PGT is indicated for patients who have a history of recurrent miscarriages, advanced maternal age (≥38 years), repeated IVF failures in spite of high grade embryos, unexplained infertility, severe male factor infertility, or inherited genetic disorders (e.g., cystic fibrosis, Tay Sachs disease, Myotonic dystrophy, etc.). Currently, there are more than 1000 types of single gene mutations that can be diagnosed.


Procedure

In order to perform genetic testing on an embryo, several cells from the trophectoderm layer are extracted on fifth, sixth or seventh day of development. This procedure is called an embryo biopsy. The extracted cells are sent out for analysis to Cooper Genomics and/or Ingenomix. Subsequently, the biopsied embryo is vitrified. After the results are received, patients will have the normal embryo transferred back.


Benefits of PGT

Patients who are carriers of single gene mutations can avoid transmitting those disorders to their offspring by testing the embryos and choosing not to transfer those which are affected or carriers.

Patients who have had several miscarriages in the past can benefit from PGT-A for aneuploidy screening by avoiding the transfer of embryos that are aneuploid and will eventually fail to implant. The likelihood of having a trisomic pregnancy increases with advanced maternal age (>38 years). PGT-A allows selection of normal embryos at the pre-implantation stage and reduces the chance of detecting abnormal fetal development during an aminocentesis in the second trimester. Patients who failed several IVF attempts and are known to generate good cohorts of embryos might have a high rate of aneuploidy and can benefit by PGT-A. Another group of patients who can benefit from PGT are those who carry translocations, which are detected by karyotyping.


Risks

There are certain risks associated with any micromanipulation procedure, but these are minimal. Trophectoderm biopsy does require embryos to develop to the blastocyst stage before the biopsy is performed. Not all couples will have embryos that develop to this stage. Occasionally, results are not able to be determined.

Maharashtra: Locals may not need CRZ nod for houses up to 3,000 sqft in coastal area

The state government has made the proposal to the Union ministry for environment, forests and climate change (MoEFCC) while forwarding the draft coastal zone management plans (CZMP) for Palghar, Thane, Sindhudurg, Raigad and Ratnagiri.

MUMBAI: Residents of coastal areas may not require permission from the State Coastal Zone Management Authority for construction of houses up to 3,000sq feet.

The state government has made the proposal to the Union ministry for environment, forests and climate change (MoEFCC) while forwarding the draft coastal zone management plans (CZMP) for Palghar, Thane, Sindhudurg, Raigad and Ratnagiri.

A presentation of the draft CZMPs based on the Coastal Regulation Zone 2021, was made to CM Eknath Shinde, who approved of the plans. The CZMPs will now be sent to the MoEFCC for its final approval.

The restriction on development has been reduced from 100 metres to 50 metres from the tide line for creeks, rivers and streams.

This will give a boost to tourism, said state environment secretary Pravin Darade who made the presentation.

Under the new notification, it will be possible to regularise old houses and create community infrastructure, said Darade.

10 Patient Safety Tips For Hospitals

 Medical errors may occur in different health care settings, and those that happen in hospitals can have serious consequences. The Agency for Healthcare Research and Quality, which has sponsored hundreds of patient safety research and implementation projects, offers these 10 evidence-based tips to prevent adverse events from occurring in your hospital.

1. Prevent central line-associated blood stream infections. Be vigilant preventing central line-associated blood stream infections by taking five steps every time a central venous catheter is inserted: wash your hands, use full-barrier precautions, clean the skin with chlorhexidine, avoid femoral lines, and remove unnecessary lines. Taking these steps consistently reduced this type of deadly health care-associated infection to zero in a study at more than 100 large and small hospitals.

2. Re-engineer hospital discharges. Reduce potentially preventable readmissions by assigning a staff member to work closely with patients and other staff to reconcile medications and schedule necessary followup medical appointments. Create a simple, easy-to-understand discharge plan for each patient that contains a medication schedule, a record of all upcoming medical appointments, and names and phone numbers of whom to call if a problem arises. AHRQ-funded research shows that taking these steps can help reduce potentially preventable readmissions by 30 percent.

3. Prevent venous thromboembolism. Eliminate hospital-acquired venous thromboembolism (VTE), the most common cause of preventable hospital deaths, by using an evidence-based guide to create a VTE protocol. This free guide explains how to take essential first steps, lay out the evidence and identify best practices, analyze care delivery, track performance with metrics, layer interventions, and continue to improve.

4. Educate patients about using blood thinners safely. Patients who have had surgery often leave the hospital with a new prescription for a blood thinner, such as warfarin brand name: Coumadin®), to keep them from developing dangerous blood clots. However, if used incorrectly, blood thinners can cause uncontrollable bleeding and are among the top causes of adverse drug events. A free 10-minute patient education video and companion 24-page booklet, both in English and Spanish, help patients understand what to expect when taking these medicines.

5. Limit shift durations for medical residents and other hospital staff if possible. Evidence shows that acute and chronically fatigued medical residents are more likely to make mistakes. Ensure that residents get ample sleep and adhere to 80-hour workweek limits. Residents who work 30-hour shifts should only treat patients for up to 16 hours and should have a 5-hour protected sleep period between 10 p.m. and 8 a.m.

6. Consider working with a Patient Safety Organization. Report and share patient safety information with Patient Safety Organizations (PSOs) to help others avoid preventable errors. By providing both privilege and confidentiality, PSOs create a secure environment where clinicians and health care organizations can use common formats to collect, aggregate, and analyze data that can improve quality by identifying and reducing the risks and hazards associated with patient care.

7. Use good hospital design principles. Follow evidence-based principles for hospital design to improve patient safety and quality. Prevent patient falls by providing well-designed patient rooms and bathrooms and creating decentralized nurses' stations that allow easy access to patients. Reduce infections by offering singlebed rooms, improving air filtration systems, and providing multiple convenient locations for hand washing. Prevent medication errors by offering pharmacists well-lit, quiet, private spaces so they can fill prescriptions without distractions.

8. Measure your hospital's patient safety culture. Survey hospital staff to assess your facility's patient safety culture. AHRQ's free Hospital Survey on Patient Safety Culture and related materials are designed to provide tools for improving the patient safety culture, evaluating the impact of interventions, and tracking changes over time. If your health system includes nursing homes or ambulatory care medical groups, share culture surveys customized for those settings.

9. Build better teams and rapid response systems. Train hospital staff to communicate effectively as a team. A free, customizable toolkit called TeamSTEPPS™, which stands for Team Strategies and Tools to Enhance Performance and Patient Safety, provides evidence-based techniques for promoting effective communication and other teamwork skills among staff in various units or as part of rapid response teams. 7 Great Ways to Encourage Your Child's Writing

 If your child struggles with writing, it’s important to find new and exciting ways to encourage her to write. Here are some creative strategies to try.


1. Write a “Convince Me!” letter

Writing isn’t just about telling stories or reporting on books. There are many ways your child can use words, including trying to convince others to agree with her. You can help her practice this type of writing by letting her argue with you—in writing! Choose a topic you don’t agree on, such as allowance or bedtime. Have your child write you a letter trying to convince you to change your mind. The catch? She has to use facts, quotes and logic to back up her argument.


2. Play a game with pictures

Photos and images are great story sparkers. Do a web search and find a few interesting images. Or cut out pictures from magazines. The pictures can be realistic, such as a photo of students on a playground. Or they can be fantastical images, such as a superhero flying in space. Glue a couple images into a notebook. Then ask your child to write about one of them. You can prompt her, asking her to include what she sees, what the people are thinking, what will happen next—or just let her imagination run free.


3. Play “Tell Me How”

In this activity your child pretends she’s writing to a space alien who doesn’t know anything about our culture. This alien does everything exactly as it’s said or written. Your child’s job is to choose an everyday task, like brushing her teeth or making a sandwich. Then, she needs to write step-by-step directions about how to do the task for the alien. When she’s done, you get to be the alien and try to follow the steps exactly as she wrote them. She may be surprised at what she left out!


4. Make an “I Can” book

As your child learns to write, she’ll also be learning other new skills. Making an “I Can” book will let her practice writing skills and keep track of her other accomplishments. Staple together a bunch of blank sheets of paper to make a book. As your child reaches a new milestone, such as learning to tie her shoes or hit a baseball, she can draw a picture on a new page of the book. Younger kids can then write, “I can tie my shoes.” Older kids can write a few sentences about what they’ve accomplished.


5. Play “Fortunately/Unfortunately”

This turn-taking writing game is based on a classic kids’ book by Remy Charlip called Fortunately. In it, something lucky happens and then something unlucky happens. Each event is introduced by either the word “fortunately” or the word “unfortunately.” To play, take a piece of paper and write a sentence beginning with “Fortunately,” such as “Fortunately it was a sunny day. I wanted to play outside.” Pass the paper to the next player, who will add an “unfortunately” sentence, such as “Unfortunately, I had to clean my room.” Keep going until the story is too silly to continue.


6. Make a journal jar

A journal doesn’t have to be a diary. It can also be a book where your child writes about ideas or answers questions, like “If you could do anything next summer, what would you choose?” A journal jar is a place to keep all those ideas and questions. Wash and decorate a wide-mouthed jar, like one that used to contain peanut butter. Then, write or print out journal prompts on slips of paper. Ask your child to pull out one prompt each day and write about it in her journal.


7. Create a family scrapbook

A family scrapbook is a great way to save memories and jump-start your child’s writing. Use an inexpensive photo album to keep souvenirs of things you do together. This can include photos, ticket stubs, and found objects, like pretty leaves.
